ストレス感受性と免疫機能―摂食障害をはじめとした精神神経免疫学的な検討―

研究概要

対象はDSM-IVで診断され、文書で研究参加への同意を得た、17例の神経性過食症(BN群)と14名の健康対照者(C群)で、全例とも女性。全例に文書で同意を得た上で、Eating Disorder Inventory、Beck's Depression Scale(BDI)、STAI、CISS、ハミルトンのうつ病および不安尺度、血漿中soluble tumor necrosis factor receptor II(sTNF-RII)、leukemia inhibition factor(LIF)を測定した。BNは23.0±3.8歳(平均±標準偏差)、C群は24.9±3.5歳、Body Mass IndexはBN群が19.8±2.5、C群が19.8±0.8といずれもt検定で有意差を認めなかった。またBN群は週に平均7.6±3.1回の過食、7.5±6.5回の嘔吐、日に7.3±15.5錠の下剤を使用(または乱用)していた。sTNF-RIIはBN群が2282±576pg/mL、C群が1941±580pg/mlとBNが高値の傾向(t=1.6,P=.11)を示したが、有意には至らなかった。LIFはBN群で3例が測定可能、C群がでは全例が測定限界以下と両群で有意な差を認めなかった。また、8症例では認知行動療法により過食症状改善後(約1ヶ月後)に再検査した。sTNF-RIIは、2478±690pg/mlが2225±619pg/mlに多少、低下したが、有意には至らなかった(t=O.9,P=.39)。また、sTNF-RIIとBody Mass Index、摂食障害、ストレスコーピング、不安、うつなどの各尺度との間に有意な相関関係は見られなかった。今後、症例数を増やし、インターロイキンの測定も行い、さらに検討したいと考えている。
