ストレス感受性と免疫機能―摂食障害をはじめとした精神神経免疫学的な検討―

研究概要	対象と方法:対象はDSM-IVで診断され、文書で研究参加への同意を得た、9例の社会不安障害(男性6例、女性3例)、20例の神経性過食症(全例女性)と20名の健康対照者(うち6名は男性)である。全例に文書で同意を得た上で、Beck's Depression Scale、State-trait Anxiety Inventory、Coping Inventory for Stressful Situations、と社会不安障害にはLiebowitz Social Anxiety Scale、神経性過食症にはEating Disorder Inventoryを施行した。免疫機能としてはPHA刺激によるLymphoproliferative response (LR)、リンパ球サブセットなどを施行した。結果:社会不安障害と健常対照群との間に、リンパ球サブセッ卜などにおいて有意な差は認められなかった。CD4数(669±296 vs. 683±151)、CD8数(464±239 vs. 508±180)、CD4/CD8比(1.3±0.5 vs. 1.5±0.6)であった。しかし、LRでは、PHA刺激あり(48149±18312 vs. 67186±27365)、PHA刺激無し(622±238 vs. 359±262)、PHA刺激あり/なしの比Stimulation Index(87±35 vs. 328±269)と2群間に有意な差を認めた(t=3.9,P=.001)。一方、神経性過食症に関しては、治療前後で2回測定した結果についてだけ検討すると、LRのPHA刺激あり/なしの比Stimulation Indexは治療前後で有意な上昇(改善)を認めた(175±143 vs. 272±240、P=.043、Wilcoxon)。一方、CD4/CD8比(1.51±0.73 vs. 1.63±0.81、P=.18、Wilcoxon)と変化なかった。考察:この様に免疫機能は状態像を反映するものと考えたれた。
