IgA腎症の発症機序の解明

研究概要	IgA腎症におけるIgA分子の異常に関する研究は既に多くの報告がなされているが、いまだ結論は得られていない。また、IgA分子の受容体であるFcαRやasialoglycoprotein receptor(AGPR)に関する基礎的研究は行われたが、ヒトIgA腎症でこれらの分子が病的意義を持つかどうかは不明である。本研究では多数例の腎生検組織像が明らかな症例からDNAを抽出し、IgA分子の産生・代謝に関わる遺伝子の多型をスクリーニングした。IgA腎症230例を含む520症例を解析し、その結果、polymeric immunoglobulin receptorをコードする遺伝子に多型がみられ、IgA腎症症例では健常対照者およびIgA腎症以外の腎炎患者に比較して、有意に遺伝子変異の確率が高いことを発見し、報告した。また、FcαR遺伝子のプロモータ領域に3カ所の一塩基多型を同定し、IgA腎症とIgA腎症以外の糸球体腎炎の患者で遺伝子頻度、haplotype頻度を比較したが、有意差を認めないことを報告した。また、腎炎進行の機序についても、レニンーアンジオテンシン系など高血圧に関連すると考えられる遺伝子や、TGF-β1やMCP-1などの炎症のメディエータの遺伝子の多型との関連を解析した。その結果、アンジオテンシノーゲン遺伝子のプロモター領域の多型が、IgA腎症の腎機能予後に影響することが明らかになった。また、ウテログロビン(Uteroglobin)遺伝子の多型が、IgA腎症の、特に尿蛋白の多い症例や高血圧のある、もともと予後が不良な症例において、予後に大きな影響を与えることを報告した。今後はマイクロサテライトマーカーと領域を絞った塩基多型(SNP)を用いて、全ゲノムを対照とした責任遺伝子の解明を行う。
