自己免疫性甲状腺疾患発症におけるFas依存性アポトーシスの役割;甲状腺免疫寛容の破綻における濾胞細胞Fasリガンド発現の関与

研究概要

Fasリガンド(FasL)は正常甲状腺濾胞細胞に発現し、甲状腺組織に浸潤するFas陽性活性化Tリンパ球にアポトーシスを誘導することから、甲状腺組織は"immunoprivileged site"であると考えられる。甲状腺濾胞細胞のFasL発現が炎症性サイトカインにより変化することが自己免疫性甲状腺疾患発症の一要因であるか否かについて検討した。
組織学的に甲状腺濾胞細胞のFasおよびFasLの発現は、甲状腺癌周囲の正常甲状腺(N)に比べバセドウ病(GD)甲状腺ではFas発現は顕著に増大しているがFasLの発現は減少していた。それに加え、GD甲状腺組織に浸潤している活性化Tリンパ球はFasおよびFasL陽性であった。GD甲状腺組織ではIL-1βの発現が強く認められるので、培養甲状腺濾胞細胞のFas/FasL発現へのIL-1βの効果を検討した。
IL-1β添加により甲状腺濾胞細胞の機能性Fas発現は顕著に増加し、FasL陽性の活性化T細胞からのFas依存性アポトーシスも強く誘導された。甲状腺濾胞細胞のFasL発現はIL-1βにより著名に減少した。甲状腺濾胞細胞に発現しているFasLは機能性であり、Fas陽性の活性化Tリンパ球にアポトーシスを誘導した。IL-1β刺激甲状腺濾胞細胞の活性化Tリンパ球への傷害性は顕著に減少していた。
以上より、GD甲状腺組織に発現しているIL-1βは甲状腺濾胞細胞の機能性Fas発現を増加させる一方で機能性FasL発現を低下させることがわかった。活性化リンパ球はFasおよびFasLが共に陽性であるので、IL-1β刺激甲状腺濾胞細胞はこれらT細胞からの傷害性は増大するが同細胞への傷害性は低下し、末梢血からGD甲状腺組織への活性化Tリンパ球の浸潤を誘導し(immunoprivileged siteの破綻)GD甲状腺組織内での免疫応答を亢進させることが示唆された。
