自己免疫性甲状腺疾患発症におけるFas依存性アポトーシスの役割:甲状腺免疫寛容破綻における濾胞細胞Fasリガンド発現の関与

研究概要	前年までの研究により、in vitroで活性化されたリンパ球は、IL-1βで刺激された甲状腺濾胞細胞に対しFas依存性アポトーシスを誘導することが確認された。バセドウ病甲状腺組織には活性化フェノタイプを有するCD4+T細胞の顕著な浸潤が認められるので、今回、これらCD4+T細胞の甲状腺濾胞細胞への傷害活性を検討した。バセドウ病甲状腺組織に浸潤しているCD4+T細胞には強いFasLの発現が免疫組織学的に検出され、単離したCD4+T細胞を用いたウエスタン解析により、バセドウ病甲状腺組織浸潤CD4+T細胞のFasL発現はバセドウ病末梢血および健常人末梢血よりきわめて強いことが確認された。機能的にもバセドウ病甲状腺組織浸潤CD4+T細胞は、IL-1β刺激甲状腺濾胞細胞への細胞傷害性を有しており、これら細胞傷害性は抗FasL抗体添加により著明に抑制された。FasLに加えFas発現もバセドウ病甲状腺組織浸潤CD4+T細胞では著明に増強していたが、これら細胞自身はFas依存性アポトーシスには抵抗性を示した。この抵抗性はバセドウ病甲状腺組織浸潤CD4+T細胞では、Bcl-2やBcl-xLなど抗アポトーシス蛋白の発現も強く検出されるためと考えられた。以上の研究結果により、バセドウ病甲状腺濾胞細胞に検出されるTUNEL陽性のアポトーシス像はFas/FasL相互作用を介し浸潤CD4+T細胞により誘導されうること、また、同組織に発現するIL-1βはこの機序を促進しバセドウ病甲状腺組織の"immunoprivileged site"破壊に関与することが示唆された。
