敗血症に伴う多臓器不全発現機構の解明-アポトーシスとの関連

研究概要

平成12年度に得られた科学研究費は、備品として遺伝子研究に必要な電気泳動装置(専用電源および泳動槽)、消耗品として遺伝子実験用試薬および電気泳動用物品の購入に充当した。今年度は、昨年度に確立したreverse transcription-polymerase chain reaction(RT-PCR)を用いたリンパ球中のP4502E1およびP4503A4の定量を中心に行った。その結果、ヒトのP4502E1活性の指標である、セブフルレンの代謝産物である血中無機フッ素の生成量と、リンパ球中のP4502E1mRNAとの間には有意な相関が認められ、リンパ球中のP4502E1mRNAと肝臓のP4502E1活性との間に相関が認められることが示唆された(Hase I,et al.Anesthesiology2000:92;1661-1666)。また、リファンピシンを投与された症例においては、コルチゾールの代謝産物である、尿中の6β-コルチゾールの排出量とともに、リンパ球中のP4503A4mRNAも増加していることが判明し、リンパ球中のP4503A4mRNAが肝臓のP4503A4活性の指標になる可能性が示唆された(Nakamoto T,et al.Pharmacogenetics2000:10;571-575)。さらに、従来から使用されている麻酔薬であるミダゾラムの代謝に関して、P4503A4が選択的に関与していることを明らかにするとともに、プロポフォールの代謝にはP4502B6が関与していることを明らかにした。これらの薬物はP4503A4およびP4502B6活性の選択的な指標として、今後有用であると考えられる。来年度以降は臓器の組織学的観察を中心に行い、敗血症モデルを形態学的側面から明らかにしたい。
