前立腺癌に対する最適な内分泌療法を推定する手法の確立

研究概要	進行前立腺癌に対する内分泌療法としては、通常持続的なアンドロゲン抑制が行われている。間欠的内分泌療法は新たな内分泌療法であり、アンドロゲン抑制による治療と治療休止とを周期的に繰り返すものである。間欠的内分泌療法の方法上の問題点として、いつ治療を休止しいつ治療を再開するべきかについて確立していない。一方、血中free/total PSAは、前立腺癌において前立腺肥大症と比較して低値を示すことが知られている。そして、内分泌療法に反応して増加することが示された。そこで、間欠的内分泌療法中に治療を再開する時期を決定する一助とするため、再燃経過中のfree/total PSAの変化を調べ、アンドロゲン不応癌への進展を避けえるfree/total PSA値を推測することを試みた。持続的内分泌療法によって治療され、連続的なtotal PSAの上昇をみた27例の再燃前立腺癌患者を対象とした。各患者より4から18検体、合計285検体の凍結血清を用いて、freeおよびtotal PSAを測定した。free/total PSAの絶対値は患者間でばらつきが大きかった。臨床的全身再燃をきたした12例中9例でfree/total PSAの減少傾向を示した。また、PSA再燃のみの11例中3例でもfree/total PSAは減少していた。さらに、間欠的内分泌療法で治療休止期にあった症例において、total PSAは3ng/ml以下と抑制されていたにもかかわらず、free/total PSAが減少傾向を示しその後全身再燃をきたした例を経験した。以上より、臨床的再燃に際してfree/total PSAは減少することが多いと推測された。したがって、間欠的内分泌療法の治療休止期間中にある患者において、free/total PSAの減少傾向がみられた場合には直ちにアンドロゲン抑制を再開すべきものと考えた。
