前立腺癌に対する最適な内分泌療法を推定する手法の確立

研究概要	アンドロゲン除去による内分泌療法後に再燃をみた前立腺癌に対するデキサメサゾン療法の有効性について検討した。デキサメサゾンの作用機序は一般に副腎抑制のためとされているが、低容量のデキサメサゾンでも効果を認めることから他の要因の関与も考えられる。そこで、デキサメサゾンの効果の機序についてさらに検討を加えた。内分泌療法施行後にPSA再燃を認めデキサメサゾンによる治療を行なった前立腺癌16例を対象とした。アンチアンドロゲン除去症候群の有無を確認後、デキサメサゾン1.5mg/dayより開始し0.5mg/dayに漸減維持した。治療効果は血中PSAの変化で判定した。デキサメサゾン療法開始前後の、血中テストステロン、コルチゾール、副腎性アンドロゲン、インターロイキン-6(IL-6)などを測定し、デキサメサゾン療法の効果との関連を検討した。デキサメサゾン療法を施行した16例中6例(37.5%)でPSAの50%以上の減少を認めた。有効期間は3-8ヶ月、平均5.7ヶ月であった。デキサメサゾン療法の有効性と、デキサメサゾン療法開始時PSA値、内分泌療法の期間、アンチアンドロゲン除去症候群の既往、血中コルチゾール、DHEA、DHEA-S、アンドロステンジオンなどとは有意な関連を認めなかった。血中IL-6はデキサメサゾン療法が有効であった4例中3例で80%以上の減少を認めた。これに対して、デキサメサゾン療法が無効であった7例においてはIL-6が80%以上抑制された例はなかった。去勢およびアンチアンドロゲンによる内分泌療法に不応となりPSA上昇をみた前立腺癌患者に対して、デキサメサゾン療法の有用性が示された。また、デキサメサゾン療法の効果の機序として、血中IL-6が抑制されアンドロゲンレセプターの活性化機構が抑制されることが示唆された。
