研究概要	異物である胎児を許容する妊娠反応は極めてユニークな生埋現象である。児を移植片と考えると、母体免疫系は児を攻撃しないように調節されており、この調節機構が破綻すると、流産、早産、妊娠中毒症等の異常が生じてくると考えられる。そこで、まず正常妊娠例におけるTh1/Th2バランスを検討し、さらに異常妊娠例で同様の検討を行なった。その結果、正常妊娠末梢血中では妊娠中期〜後期にかけて緩徐にTh2優位に変動することが明らかとなった(文献1)。一方、母子接点の場である子宮内膜では、排卵前ではTh1が優位(Th1/Th2比:147.5±96.7)であったが、排卵後にTh1/Th2比は37.4±21.3と減少し、妊娠初期ではその比が1.3±0.5と急激にTh2優位に変化することを見出した(文献2)。すなわち、着床局所において児を攻撃するTh1細胞が減少し、その結果妊娠が維持されているものと考えられた。次に妊娠中毒症例での末梢血より単核球を分離し、PHAで刺激後にTh1型サイトカイン(IL-2.IFNγ.TNFα)とTh2型サイトカイン(IL-4)を測定すると、妊娠中毒症例ではT細胞が活性化をうけており、Th1型サイトカイン産生が高まり、逆にTh2型サイトカイン産生が低下してくることが明らかとなった。さらに妊娠中毒症例での重症化の指標である平均血圧と、産生されたIL-2.IFNγ.TNFαとが有意な正の相関を示すことも明らかとなった(文献3)。また妊娠中毒症例末梢血中のTh1/Th2比を検討すると、正常妊娠例に比し、有意に高値となり、Th1優位となることも明らかとなった(文献1)。以上によりTh2優位になることで妊娠は維持されるが、このバランスがTh1に傾くと妊娠中毒症のような異常が生じることが初めて明らかとなった。
