アジア地域の環境における腸管感染症原因菌の動態に関する調査

研究概要

(1)我々が発見した腸炎ビブリオの新型クローンの世界的大流行の動態について詳細に研究した。主たる血清型であるO3:K6型以外のO4:K68、O1:KUT血清型が新型クローンに属することを支持するDNA解析結果(ribotyping法、パルスフィールド・ゲル電気泳動法)が得られた。また、タイ南部のハジャイの海産物からO3:K6血清型に属する新型クローンを分離したので、この地域の2つの病院で1999年1年間に下痢患者から分離された腸炎ビブリオ323菌株を解析した。GS-PCR法による検査の結果、76.3%が新型クローンに属し、その中に既知の3種の血清型以外の新たな3種の血清型に属する菌株が発見できた。AP-PCRプロフィール、線状ファージゲノム塩基配列、およびtoxR遺伝子の塩基配列の解析からもこれらの新血清型菌が新型クローンに属することを確認した。さらに3種の新血清型のうち2種が、海外旅行者から最近分離菌された新型クローン菌株の中に発見できた。したがってO3:K6新型クローンの一部はアジアの環境中で表面抗原を変化させた種々の派生型に変化しつつ感染症をおこして、少なくとも旅行者によって世界へ伝播されていると言える。今後はタイの環境中にO3:K6血清型以外の血清型に属する新型クローンが分布することを示し、この仮説を証明する予定である。一方、わが国へ輸入される魚介類を18ヶ月間検査した。腸炎ビブリオは多くの検体から分離できたが、tdh遺伝子陽性またはtrh遺伝子陽性菌株は1〜2%の検出頻度であった。いまのところ新型クローンに属する菌株は検出されていないが、検査を続行する予定である。
(2)前年度までに東南アジアの環境中に病原性大腸菌O157が分布することを証明したので、タイでこの菌に感染した患者を探している。今のところ発見されていないが、検査を続行中である。
