抗マラリア剤効果と人間、原虫、媒介蚊の遺伝的多様性に関する共同研究

研究概要

我々は、南太平洋のマラリア流行地であるヴァヌアツにおいて、これまで白人では約3%、アジア人では13-23%と報告されていた肝cytochromeP450(CYP)のisozymeであるCYP2C19に関するpoor metabolizer(PM)遺伝子型の頻度が、71%と極めて高いことを既に報告した(kaneko et al.1997Lancet)。今年度はヴァヌアツのPG治療後マラリア患者から得られたサンプルについて、CYP2C19遺伝子型とPG代謝の関係を解析した。更にヴァヌアツの主要島嶼住民のCYP2C19に関するgenotypingを行い、CYP2C19変異遺伝子分布の全体像を明らかにした。以下が結果の要約である。
1)PG300mg/day、3日間経口投与において治療されたP.falciparumないしP.vivax患者95名中、33名がCYP2C19遺伝子型に関してextensive metabolizer(EM),62名がPMであった。血中PG濃度は両者で同様であったが、有為なCG濃度はEMにおいてのみ検出された。EMは更に野生型遺伝子の数でヘテロとホモの2群に分けられたが、ヘテロEMのPG代謝の程度はホモEMとPMの中間であり、"gene dose effect"の存在が示唆された。2)16島24地域の住民5538名について、CYP2C19に関する遺伝子型を決定した。PMの比率は38-79%とばらつきを認めたが、全体で61%であった。CYP2C19^*1(野生型)、^*2、^*3の遺伝子頻度はそれぞれ0.223、0.633、および0.144であった。これら地域の南緯度とCYP2C19に関するHeter ozygosityの間には有意な相関を認め、更に地域間の遺伝的距離と言語的および地理的距離の解析の結果は、現在のCYP2C19に関する変異遺伝子の分布には短距離のgene flowが重要であることを示唆した。
この結果の意義についてアジア・太平洋の研究者、WHO専門家と協議し、今後更にPGを含む複合投与法について研究を推進するためのマラリア疫学調査を2000年3月にタイ、ヴァヌアツで行う。
