抗プロトロンビン自己抗体の血栓形成機序に関する病態生理学的検討

研究概要	プロトロンビン(PT)は主要な抗リン脂質抗体の対応抗原である。抗PT抗体は照射ELISAプレートにPTを固相化するか(aPT)、またはホスファチジルセリンにPTを結合させて抗原とする(aPS/PT)ことによって検出できる。はじめに自己免疫疾患患者のaPTとaPS/PTを測定し、その特徴を検討した。患者265人を対象にIgG aPT、aPS/PTを測定し、抗リン脂質抗体症候群(APS)の臨床病状およびループスアンチコアグラント(LA)の有無と比較した。aPT、aPS/PT両者陽性は8人(1群)、aPTのみ陽性は20人(2群)、aPS/PTのみ陽性は16人(3群)であった。各群でのAPS/LAの頻度は順に50%/75%、16%/21%および50%/81%であり、2群ではAPS、LAとも頻度は低かった。次に、陽性サンプルは塩抵抗性も検討した。0.5M NaClの存在化での結合を2群のaPTと3群のaPS/PTで比較したところ、2群のaPTが有意に低かった。また血中PT濃度を抗原量および凝血学的活性で測定したが、いずれの抗体価とも関連はなかった。以上より、抗PT抗体の意義は測定法によって異なることを明らかにした。aPS/PTとaPTで検出される自己抗体は、少なくとも多数の血清サンプルで同一の抗体ではなく、aPS/PTはaPTに比べてAPSやLAの存在とよく相関していた。aPS/PTはaPTより塩抵抗性が強く、affinityの高い抗体である可能性が考えられた。抗体価とPT濃度との相関の欠如から、抗PT抗体は単なる中和抗体でなく、低PT血症の原因とならないことが示された。 aPS/PTおよびaPTの免疫学的性質と凝固系への作用を詳細に検討するため、balb/cマウスをヒトPTで免疫し、モノクロナールaPS/PTを作成し、すでに数クローンを樹立した。現在その特徴を解析中である。
