研究概要

アジア13カ国、欧州4カ国、南アフリカ、南アメリカ2カ国、オーストラリア、及び米国から、1998-1999年に臨床検体から検出されたMRSAにつき、グリコペプチド系(バンコマイシン、テイコプラニン)、βムラクタム系(メチシリン、アンピシリン、イミペネム、セフチゾキシム)、その他の系統の抗生物質の感受性検査を行った。その結果、韓国、タイ、日本、ブラジル、ガテマラ、南アフリカ、スコットランド(英国)、フランス、米国からバンコマイシン低感受性菌(VRSAおよびhetero-VRSA)を検出した。さらに、これらの菌株の分子疫学マーカーの検討を行い、バンコマイシン耐性を生じやすい菌株の特定を行った。その結果、日本に蔓延しているMRSAのクロノタイプII-Aが、世界各国で分離された11株のVRSAの7株を占めることを見出した。さらに、韓国、米国、日本、ブラジルにクロノタイプII-AのMRSAが多く分布しており、これらの国では、バンコマイシンの感受性が低下傾向にあることがわかった。さらに、クロノタイプII-Aのバンコマイシン耐性機構を生化学的に研究し、細胞壁合成の増加が関与していることをあきらかにした(投稿中)。また、このクロノタイプの持つ多剤耐性を決定する巨大な(52-kb)DNA領域の全構造を解明し、これが、新規のmobile DNA 、(SCCmecと命名)、であることを明らかにした(印刷中)。このSCCmecの塩基配列は、MRSAの新しい、分子進化を考慮にいれた疫学マーカーとしてきわめて有用であり、MRSAの新規タイピング法を開発中である。
