研究概要

すでに報告した日本のMRSA分離株N315のメチシリン耐性を規定する領域は、新規の組み換え酵素(CcrA,CcrB)の働きにより染色体上に挿入し、また正確に切り出される新規の動くDNAであることを証明し、このDNAをSCCmecと命名した。タイ研究者との共同研究で、世界20カ国のMRSA分離株を解析し、SCCmecには異なる3種類があること、その塩基配列の違いを基礎にプライマーを設計することにより、世界各国に分布するMRSAのクローンの推定をすることができることを示した。バンコマイシン耐性菌のスクリーニングにより、韓国の研究者と共同で、同国のバンコマイシン治療が奏功しなかった患者からMIC8mg/lを示すVRSA(=VISA)を見いだし報告した。また、タイの2つの病院からそれぞれバンコマイシン低感受性株(hetero-VRSA)を見いだし報告した。日本のバンコマイシン耐性株Mu50の耐性メカニズムは細胞壁合成亢進による細胞壁肥厚が関与しており、その直接のメカニズムとしてaffinity trapping,clogging of peptidoglycan layerという2つの要素を提唱した。Hetero-VRSAによる感染症治療には、単剤として有効な抗菌薬が存在しないため、併用療法が世界各国で行われている。本年は、日本のhetero-VRSA株につき、我が国でMRSA感染症治療に頻用されているバンコマイシンとβ-ラクタムの併用効果を検討したところ、MICに近い薬剤濃度ではわずかなadditive effectが見られたもののsub-MICではむしろ検討した7つのβ-ラクタムすべてに拮抗現象が見られた。以上、アジア地域に少なくとも2つの異なったMRSAのクローンが蔓延していること、それぞれから異なる耐性メカニズムを持ったglycopeptide耐性株が生じつつあることを示した。日本における低感受性株に関しては、その治療上、バンコマイシンとβ-ラクタムの併用は、慎重を要することを示唆する研究結果を得た。
