感温性色素素材を用いた老年者閉塞性動脈硬化症の診断に関する研究

研究概要	温度変化により変色する衣料用色素素材を組み合わせて、摂氏19度から42度まで0.5度刻みに四肢体表皮膚温を可視的に判別可能なテープ素材を開発した。このテープ素材を用いて、高齢者の皮膚表面温度を測定し、末梢血管障害の診断における本素材の有用性につき検討した。対象は高齢者235例(男111例、女124例、80±9歳)で、下肢470肢感温変色性色素テープおよびサーモグラフィーによる皮膚表面温度、および上肢下肢血圧比(ABI)を比較した。足背における感温変色性色素テープおよびサーモグラフィーによる測定温度はr=0.46,p<0.0001の良好な正相関を示した。また感温性色素テープにて測定した体幹部と足背の温度差はABI<0.5の例において7.45±3.07℃とABI【greater than or equal】0.5の例の6.31±2.76℃と比較し有意(p=0.019)の増大を示した。ABI<0.5および感温性色素テープでの体幹部と足背の温度差7℃以上の例での下肢閉塞性動脈硬化症の診断の一致率はχ2検定でp=0.0044と有意の一致性を示した。以上のことから、感温変色性色素素材による下肢皮膚温測定は下肢閉塞性動脈硬化症の診断に有用である。閉塞性動脈硬化症は高齢者に多発するが、サーモグラフィーは高価で一部の施設でしか使用されていないのが現状である。また、ABIの測定はある程度の熟練を要することから、実際日常臨床には頻用されているとは必ずしも言い難い。本素材による下肢皮膚温測定は日常臨床に簡易に使用しえ、医師ばかりでなく、看護婦、あるいは介護者によっても、本症の存在を予見し、要介護老人が著増する我が国の本症の早期発見、早期診断、早期治療に資する。
