ナトリウム代謝感受性遺伝子多型および変異の検査同定法の確立とその臨床応用

研究概要	今回の検討では、ナトリウム代謝感受性遺伝子について臨床的および基礎的検討を行った。臨床的検討では、ナトリウム代謝感受性遺伝子として重要と考えわれるCYP11B2遺伝子のT-344C多型およびIC多型と高血圧との連関解析を行った。 CYP11B2 T-344C多型の遺伝子型は高血圧群でC対立遺伝子の頻度が低くTC+CC遺伝子型が健常対照群の67%に比し56%を占めていた。T-344C多型は高血圧症と有意に関連していた。(p=0.027;odds ratio,0.6;95%confidence interval,0.6-0.9)。一方、IC多型は遺伝子型および対立遺伝子とも、高血圧群と健常対照群の両群間に差は認められなかった。以上の結果から、東北地方における高血圧の遺伝的危険因子としてCYP11B2 T-344C多型が関与している可能性があり、遺伝子マーカーとして臨床応用の必要性が示唆された。引き続きアミロライド感受性Na-K-2Cl共輸送体の遺伝子多型との連関解析を施行中である。一方、基礎的検討では、サイアザイド感受性NaCl共輸送体遺伝子非翻訳領域上流の単離・機能解析から糖質コルチコイドによる調節の可能性を見い出している。また、Gitelman's症候群で見い出されたL623P変異の変異体組み換えクローンによる機能解析を試みている。
