慢性関節リウマチの多因子遺伝に関わる疾患遺伝子の同定

研究概要	慢性関節リウマチの遺伝素因について、家系のマイクロサテライトマーカー解析を用いて、第1染色体D1S214/253、第8染色体D8S556、X染色体DXS1232の3箇所にRAの疾患感受性遺伝子座位RA1、RA2、RA3を同定し、さらにこの中でRA1遺伝子としてdeath receptor DR3遺伝子を、またRA3遺伝子としてDbl遺伝子のDNA上での変異を特定した。 RA1遺伝子すなわちdeath receptor3(DR3)遺伝子はFasファミリーの一員で、アポトーシス誘導受容体である。RA2遺伝子は、DR3エキソンマップ上におけるnt564(A→G);Asp159Gly,nt630+622(del14),nt631-538(C→T),nt631-391(A→T),nt631-243(A→G)のSNP4箇所および核酸欠損1箇所の変異体であった。変異のために転写が早期に終結し、細胞内death domainを持たないDR3変異体が生成した。DR3変異分子は正常DR3分子と三量体を形成し、ドミナントネガティブにDR3の機能を低下させた。DR3変異体存在下にDR3分子にはTRADD分子が結合せず、下流のCaspase8以下のシグナル伝達が阻害された。変異は多発家系のRA例で6/60(10%)、孤発RAで7/297(2.36%)、健常対照者で1/266(0.38%)に認められ、ベイズ定理によると家系で発端者が当該遺伝子によって発症する確率は96%以上と計算された。DR3は主にリンパ系に発現するから、自己免疫自然発症MRL/lprマウスのFas欠損の場合と同様に、増殖したリンパ系細胞が適切にアポトーシスに陥らないために自己免疫状態に陥ると考えられた。一方RA3遺伝子は、Dbl遺伝子3'端近くの223bp(第23,24エキソン)のエキソンスキッピング変異で、この分子的基盤としてDNA上にnt2522+394(C→T)およびその他のSNPが見出され、RA患者の40%に存在していた。Dblは低分子量G蛋白Rac、cdc42を支配するが、Dbl変異体は下流のRac-2に結合するがこれを活性化できない。RacはNADPHオキシダーゼの構成成分であり、fMLP刺激に対する好中球の活性酸素生成能はDbl変異例において有意に低下していた。また、Dbl変異体のcdc42に対するGEF(GTP exchange factor)活性が30%低下しており、今回見出された好中球の異常が遺伝素因としてRAの発症病因に関わるとする今回の知見は好中球の重要性を示唆している。
