橋本病における疾患感受性遺伝子の同定

研究概要	Graves病(GD)および橋本病(HT)は、両者ともに甲状腺特異的な自己免疫疾患でありながら、その臨床的および免疫学的所見は異なること、それぞれの多発家系の存在、twin studyより、CDおよびHTのそれぞれに特異的な遺伝要因の関与が示唆される。一方、GDとHTの混在する家系、GDからHTに移行する症例が存在することにより、両者に共通の遺伝要因も存在すると考えられる。本研究では、HTがどのような疾患感受性遺伝子を特異的に持ち、どのような疾患感受性遺伝子をGDと共有することにより自己免疫性甲状腺疾患(AITD)を形成するのかを明らかにすることを目的とした。書式化されたインフォームドコンセントのもとに収集されたAITD113家系の罹患同胞対法123組(うちGD家系67、HT家系25組、GD-HT混在家系31組)に対して、約400個のマイクロサテライトマーカーを用い、全ゲノムスキャンを行い、罹患同胞対法を用いた連鎖解析を行った。統計学的処理は、MAPMAKER/SIBSによるmultipoint maximum lod score(MLS)の算出と、SIBPALによるP valueの評価で行った。HTではD8S272でMLSが3.768、P valueが0.0002を示し、AITD全体ではD5S436でMLSが3.141、P valueが0.0017を示した。AITDおよびHTの疾患感受性遺伝子が5q31-33、8q23-24にそれぞれ存在することが示唆された。現在、孤発例も収集し、示唆的な領域において100kb間隔でマイクロサテライトマーカーを設定し、association studyを進めている。この研究は、九州大学に設置されたミレニアムゲノム倫理審査委員会により承認された。
