研究概要

本研究の特徴は、正常者及び神経疾患患者を対象として、各種の感覚モダリティーの脳内情報処理過程と、その出力としての随意運動の準備・遂行過程を、複数の非侵襲的方法を統合的に用いて解明することであり、以下のような業績が得られた。
(1)磁気共鳴機能画像法(fMRI)に随意的筋弛緩課題を適用して、局所性ジストニー患者で運動皮質における抑制機構の異常を示唆する結果を得た(Oga T., et al., in press)。健常者を対象とする研究で得られた先端的な脳研究の知見を積極的に臨床応用することは、社会的波及効果も大きいと予想される。
(2)難治てんかん患者の術前検索の一環として、刺激間間隔2秒の視覚対刺激を与えて第1刺激を選択課題とする遅延反応課題を用い、皮質電位を記録した。その結果、まず運動前野(PM)の腹側・吻側に刺激受容に関連した活動が起こり、続いてPMの背側、さらに尾側の一次運動野に向かって活動が移行した(投稿中)。ヒトの感覚運動機能連関におけるPMの意義の一端を明らかにした。同様の遅延反応課題を脳磁図記録によって正常被験者で検討することで、脳機能の詳細を非侵襲的に解明する実験を行っている。
(3)随意運動遂行時の脳活動部位間の機能連関を明らかにするために、脳の電磁気的律動性活動のコヒーレンス解析を行った。難治性てんかん患者の術前検索の一環として行われた硬膜下記録脳波の解析によって、運動の直前に一次感覚運動野と補足運動野との間の機能的連関が増強することを発見した(Ohara et al.,2001)。これは、複数の運動野のネットワークの変化が、運動統御に重要な役割を果たしていることを示唆する。
(4)経頭蓋的磁気刺激を用いた研究手法の開発についても、実験計画については当施設での倫理委員会の承認を得て、基礎的実験および臨床応用をおこないつつある。
