体幹部定位放射線照射を目的とした体幹部固定装置の開発のための基礎的研究

研究概要	本研究では、頭蓋内病変では標準化しているラジオサージャリーの技術の体幹部腫瘍への応用をめざし、具体的には定位放射線照射用体幹部固定装置を用いた場合の固定精度の検証、三次元放射線治療計画における最適な安全域の検討、呼吸性体動に対応した最適な放射線照射法の検討を行うことによって、最適な体幹部固定法について検討した。また、個々の患者の病変部位と患者条件に基づいた治療計画を行うことにより、高精度に体幹部腫瘍に対する三次元定位放射線照射を実現することを目的とした。定位放射線照射用体幹部固定装置を用いた場合に、体幹部病巣の固定精度は直線距離で0-8.5mm(平均2.5mm)以内に保たれており、十分実用的であることが検証された。また、腹部圧迫呼吸抑制器具を用いることによって、孤立性肺腫瘍の呼吸による移動距離が圧迫前の8-20mm(平均12mm)から2-11mm(平均7mm)に有意に(p=0.002)制限されることが証明された。しかし、これらの検討を通じて、固定装置の問題点も明らかにされ、固定具側面形状の変更や垂直微調整機能の追加などの今後の改良の必要な点が明らかになった。また三次元放射線治療計画に基づいた安全域についても、水平方向5mm、垂直方向8-10mmとして治療計画を行った結果、中央観察期間が12ヶ月であるが局所制御率が96%と非常に高度であり、かつ有症状の合併症も認められず、非常に有用な治療法である可能性が証明された。現在までの体幹部腫瘍に対する定位放射線照射の初期経験を通じて、最適な体幹部の固定法、呼吸移動に対する対処法、最適な定位放射線照射分割法などについてはまだまだ解決すべき問題点があり、今後も最適な体幹部定位放射線照射の実現に向けての検討が望まれる。
