研究概要

(1)遺伝子発現解析による乳癌の予後診断:乳癌予後の新しい診断法として、遺伝子発現プロファイル解析の有用性を検討すべく、以下の検討を行った。まず、原発乳癌98例を対象として2500遺伝子の発現をATAC-PCR法で解析し、エストロゲン受容体(ER)と有意に相関する21遺伝子を抽出した。次に、この21遺伝子の発現情報を基にクラスター解析を実施すると乳癌を3つのグループ(I、II、III群)に分類することができた。この分子分類の妥当性を更に203例の乳癌を追加して確認した。その結果、II、III群は、I群に比して有意に予後が不良であり、かつ、この分子分類は古典的な予後因子(リンパ節転移、腫瘍径、ER、組織学的異型度)とは独立した予後因子であることが明らかとなった。現在、更に症例を追加してprospective studyを実施している。
(2)遺伝子発現解析による乳癌の化学療法感受性の診断:乳癌の化学療法として汎用されているdocetaxelの効果予測診断法を開発すべく以下の検討を行った。即ち、術前化学療法施行予定の乳癌症例を対象として、まず、遺伝子診断用の腫瘍サンプルを採取し、その後に化学療法(docetaxel)を実施した。docetaxel療法施行前に採取した腫瘍組織(n=47例)における遺伝子発現プロファイル(2412遺伝子)をATAC-PCR法で解析し、化学療法有効例と無効例における遺伝子発現プロファイルの差異から、85遺伝子の発現情報に基づく効果予測診断モデルを構築した。次に、その妥当性を新たに集積した23例を対象としてprospectiveに検討した結果、正診率は81%と良好であった。現在、docetaxelと同様に乳癌治療に汎用される抗癌剤であるanthracyline系抗癌剤についても同様の検討(効果予測モデルの作成)を実施している。
