体系的遺伝子異常解析に基づく乳癌・甲状腺癌の遺伝子診断法の開発

研究概要	「体系的遺伝子発現解析」cDNAマイクロアレイ解析実験の条件検討を詳細に行い、組織RNAの検定、T7RNAポリメラーゼによるRNA増幅の条件設定をし、ヒト遺伝子の約2/3を一度に解析できる25,344種のcDNAを高密度にスポットしたマイクロアレイの作成を行い、ハイブリダイゼーション、スキャニング・データの数理解析の実験条件を最適化し、種々の癌組織における発現プロファイル解析を行った。乳癌では1995年から1997年の2年間で癌研究会附属病院で根治術を受けた約1,000症例の新鮮凍結標本を用いて(1)n0乳癌(リンパ節転移陰性乳癌)の中で5年再発群と5年健存群間の体系的遺伝子発現の相違の比較検討、(2)ER(エストロゲン受容体発現)陰性乳癌の中での5年癌死群と5年生存群間の体系的遺伝子発現の相違の比較検討、(3)T2(腫瘍径2cm以上5cm未満)進行乳癌の中での5年癌死群と5年生存群間の体系的遺伝子発現の相違の比較検討、(4)n1β<(リンパ節転移4個以上の高転移群)乳癌の中での5年癌死群と5年生存群間の体系的遺伝子発現の相違の比較検討という臨床応用において極めて重要な課題についてcDNAマイクロアレイを用いた体系的遺伝子解析を用いてそれぞれに実験を行った。各検討において体系的遺伝子発現の相違をMann-Whitney testおよびRandom permutation testを用いて統計解析を行った。発現異常を認めた各遺伝子群に関してはRT-PCR法にてその発現を確認した。さらに発現確認できた遺伝子群を組み合わせることにより予後予測システムの構築を試みた。RT-PCBで特に二群間(再発群と健存群または癌死群と生存群)で発現差異の大きい遺伝子をn0乳癌においては10個組み合わせスコア化(各遺伝子の発現比=各遺伝子の発現量/各遺伝子のGAPDHの発現量)することにより、再発群と健存群が有意差を持って群別できる再発予後予測可能な結果が得られた。同様にRT-PCRで特に二群間(癌死群と生存群)の中において発現差異の大きい遺伝子をT2進行乳癌で10個。ER陰性乳癌では20個の発現異常を認めた遺伝子群をそれぞれ組み合わせスコア化することにより、各臨床条件における予後予測可能である良好な結果を得られた。さらにT2進行乳癌ER陰性乳癌においてはマイクロアレイに用いた症例とは別に新たに症例を加え追試を行ったが同様の結果を得られた。甲状腺乳頭癌の遺伝子診断の開発に関して、約2万5千種のcDNAマイクロアレイの結果から、甲状腺乳頭癌の発現プロファイルを作成した。解析により、癌組織内で出発現量の増加が見られた遺伝子が152個、同じく低下が見られた遺伝子が18個、確認された。それらの遺伝子のうち12個を選択し、RT-PCR法により癌組織と正常組織での発現を各々確認し、発現量の変化の程度と頻度を検討した。また、遺伝子産物である蛋白質にたいする抗体を用いて、その免疫組織染色法を用いて甲状腺組織におけるその発現部位および発現状況を検討した。
