消化器がんの進展に関連する分子と治療予後に関する研究

研究概要	がんの制圧を阻害する最大の要因はがんの浸潤、転移である。その機序について多くの研究がなされてきた。本研究においては、我国のがんの60%以上を占める消化器癌について、がん浸潤、転移に関連する分子あるいは遺伝子の中から、臨床病理学的な検討を経て、治療選択や予後とも密接に関連する生体マーカーを見い出し、詳細かつ大規模な臨床疫学的研究を実施し、がんの制圧、予後の改善に貢献する。具体的には、マトリックスメタロプロテアーゼの一種マトリライシン、がん制圧遺伝子と考えられるDCC,KAI-1およびムチン分子について、癌の進展と予後あるいは治療選択の指標としての意義を明らかにする。その結果、マトリライシンはがん浸潤、転移に関連する重要な分子であるばかりでなく、早期大腸癌の浸潤先端部に発現し、かつ予後にも影響を及ぼしている可能性が本研究により明らかにされた。また、DCC mRNA発現低下の検出が、胃癌再発の予測因子となることがはじめて明らかとなった。さらに、食道の前癌病変とも考えられるバレット上皮に大腸型ムチンの発現が高頻度に認められ、この発現と発癌が密接に関連することを示し得た。この変化が治療の選択に役立ち得るかなどについて、さらに研究を進める予定である。
