研究概要	本研究の目的は、発がんリスクの個体差の理解とそれに基いたがん予防を実現することである。そのため、環境だけでなく宿主要因に注目した発がん高危険群の選別とがん予防に有用な生体マーカーを開発することが必要である。まず、埼玉県住民を対象としたコーホート研究により、末梢血リンパ球のNK活性と発がんとの関連をすでに報告しているが、NK活性の個人差を規定する遺伝的要因を見出した。すなわち、NK活性が低い健常群と高い健常群を比較する関連解析により、NK細胞の主要な活性型受容体であるNKG2Dの遺伝子領域にNK活性と強く関連した2つの主要なハプロタイプを同定した。これらのハプロタイプは日本人での頻度も高く、大きな個人差を示すNK活性の主要な遺伝的要因であると考えられる。また、NK細胞は周囲の細胞とHLAクラスIを介して、その恒常性を維持していると考えられるため、HLAクラスI遺伝子型とNK活性を比較した。その結果、HLAクラスIの4つの遺伝子型がNK活性と有意な関連を示し、NK活性の個人差にも関与していることを示唆した。コーホート研究と並行して、臓器別にがん予防に有用な生体マーカーの研究を進めてきた。胃がん:H.pyloriが遊離する新しい発がん因子Tipの研究を進め、Tipは菌体中でホモダイマーを形成して遊離されるが、モノマーは発がん活性を示さないことを見出した。Tipのダイマー形成阻害は、胃がん予防の新しい戦略となると考えられる。乳がん:HDAC6やIGFBP4などの新規予後因子を見出した。肺がん:肺がん患者の血漿hnRnP B1 mRNA量が、良性肺疾患患者、健常人に比べて有意に高いことを見出した。hnRNP B1は気管支異形成でも高発現することから、前がん病変のマーカーとしても有用である可能性がある。
