研究概要

アジア地域における消化管がんの予防対策の推進に資するため、北東アジア三国(日・中・韓国)が協力し、消化管がんの各国・地域における共通要因と特異的要因を探索する民族疫学研究を1999年度から5年計画で展開している。今年度まで三国6地域(日本、韓国、中国4地域)の中核病院において各国・地域で開発した半定量的食物摂取頻度調査票(SQFFQ)を用い、大腸がん・胃がんの症例対照研究のための資・試料収集中である。そこでは、一般的な食・生活習慣に関する情報収集、遺伝子多型を検索するためのDNA材料、および血清脂質をはじめとした生化学的検索のための血漿・赤血球などを収集している。
今年度までの各地域における研究達成度は、1)愛知県:県がんセンター病院の初診患者約10,000名分(2,000例のがん患者以外が比較対照群)の中で目標値の胃がん、大腸がん患者各400例の調査を終了、2)韓国:ソウル国立大付属病院では大腸がん患者群500例と同数の性・年齢をマッチングさせた比較対照患者群、3)中国:江蘇省で大腸がん症例群と同地域に居住する比較対照群300対、四川省重慶市の第三軍医大学で大腸がんの症例・対照群を各300対、遼寧省では研究開始が遅れたので症例・対照群を各200例、広東省汕頭市の大学病院では調査を開始した。
来年度は調査を終了した各地域で疫学的解析を開始するが、日本で前解析した大腸がんの分子疫学的研究の成果によると、大腸がんの危険要因として獣肉類の頻回摂取と多量飲酒習慣、防御要因として健康のための定期的運動が上げられた。特に、肉類の摂取についてはPPARγ(節約遺伝子)の活性型(欧米人よりアジア人で頻度が高い)で、飲酒習慣についてはALDH2の不活性型で危険度が倍加するという知見が得られ、日本人の欧米型食嗜好が大腸がんの危険度を高める可能性が疫学的に示唆された
