研究概要

泌尿器系疾患でも多くのcommon diseaseが含まれている。その発症原因は不明である。そこで泌尿器系でもcommon diseaseの範疇に入る疾患に診断基準を作成して分類し、各疾患の組織のDNA、RNA、血液DNA検体を蓄積し、疾患の原因と考えられる候補遺伝子群の遺伝子多型解析を行う事は今後泌尿器系疾患の病院解明のため重要な検討課題である。研究目的は泌尿器系の中からcommon diseaseにあたる疾患の血液DNA,組織のRNA検体収集を行い、それら疾患の発症に関わるとされる原因遺伝子群の遺伝子多型(SNP)との関連性を検討する事である。我々はまず、前立腺肥大症、尿路結石、尿路性器癌(前立腺癌、尿路上皮癌、腎細胞癌、精巣腫瘍)を対象とし、各疾患で300検体を収集しこの解析に当てるという内容であった。
本研究の中核の内容である血液DNAの収集については現在までのところ腎癌・・81件、膀胱癌・・145件、前立腺癌・・176件、精巣腫瘍・・20件、前立腺肥大症・・116件、尿路結石症・・105件、膀胱尿管逆流症:7件、尿道下裂・・・:28件、その他疾患・・:72件、正常・・・・・:37件、の収集が昨年末までに得られている。しかしながら初期の目標検体数にはまだ達していないため現在も進行中である。
Microarray解析は腎細胞癌の転移、paraneoplastic syndromeの有無、膀胱癌の抗ガン剤抵抗性、腎細胞癌のVHL遺伝子変異の部位による悪性度の違い、などのいくつかのテーマで進行中である。現在のところはarrayの解析結果がまだ得られていないため報告が不可能である。
以上のように計画の2年間では初期の目的に達しなかったため、今後さらに1年間の追加期間で研究を継続する。
