自己免疫性甲状腺疾患(橋本病)の疾患感受性遺伝子の同定とその機能解析

研究概要	Graves病(GD)および橋本病(HT)は、両者ともに甲状腺特異的な自己免疫疾患でありながら、その臨床的および免疫学的所見は異なること、それぞれの多発家系の存在、twin studyより、GDおよびHTのそれぞれに特異的な遺伝要因の関与が示唆される。一方、GDとHTの混在する家系、GDからHTに移行する症例が存在することより、両者に共通の遺伝要因も存在すると考えられる。本研究ではGDおよびHTが1)どのような疾患感受性遺伝子を共有し、2)どのような疾患感受性遺伝子を特異的に持つことにより、自己免疫性甲状腺疾患(AITD)を形成するのかを明らかにすることを目的とする。書式化されたインフォームドコンセントのもとに収集されたAITD113家系の罹患同胞対123組(うちGD家系67、HT家系25組、GD-HT混在家系31組)に対して、約400個のマイクロサテライトマーカーを用い、全ゲノムスキャンを行い、罹患同胞対法を用いた連鎖解析を行った。統計学的処理は、MAPMAKER/SIBSによるmultipoint maximum lod score(MLS)の算出と、SIBPALによP valueの評価で行った。AITD全体では、D5S436でMLSが3.141、P valueが0.0017を示した。また、HTについては、D8S272でMLSが3.768、P valueが0.0002を示した。AITDおよびHTの疾患感受性遺伝子が5q31-33、8q23-24にそれぞれ存在することが示唆された。現在、罹患同胞対法の数を増して連鎖解析を行うとともに、孤発例も収集し、示唆的な領域において新たなに100kb間隔でマイクロサテライトマーカーを設定し、相関解析を進行させている。
