急性前骨髄性白血病における微少残存腫瘍とレチノール酸耐性に関する前方視的研究

研究概要

APL患者のキメラ遺伝子解析による微少残存腫瘍(MRD)モニタリングおよびレチノール酸耐性の前方視的研究を、日本の主要小児がん研究グループ(CCLSG,TCCSG,JACLSおよび東北グループ)の共同研究として行っている。定性的PCRによるMRDモニタリングを3ケ月毎に行い、初発時および経過中陽性検体は定量的MRD評価(TaqMan PCR)も行うことで「キメラ遺伝子陽性の"molecular relapse"臨床血液学的再発の事前予測に有用であるか」を検討する。1997年12月以降約20例のAPL患者をモニターしているが、再発が2例のみ(骨髄外再発1、off-therapy後1年しての再発1)で有用性の判定には研究継続が必要である。また、定量PCRの感度が低く量的変化の評価範囲が狭い(細胞混合比で10-2レベルの検出感度)ため、臨床応用は現時点では難しい。一方、初発時骨髄の解析で、PML/RARaキメラ遺伝子short formの発現量がlong formの約10倍であるが判明し、short form APLが予後不良因子であることとの関連が示唆された。
ATRA耐性患者から樹立されたAPL細胞株(UF-1)はmutant PML/RARa(RARa/E-domainにArg276Trp)を有し、in vitro細胞培養では1uM ATRAに反応して分化とアポトーシスを起こすが、100nM以下のATRAには全く感受性を示さない。一方UF-1は、RARaおよびRXRの双方にリガンドとして結合する9-cis-RAに対しては1uMのみならず100nM以下の濃度でも容量依存性に感受性を有する。さらに、RXR特異的アゴニスト(HX600)は100nM ATRAと併用するとUF-1のATRA感受性を回復させることがら、UF-1のATRA不応性がRXRの活性化により回復することを示唆する(Sato A, Imaizumi M et al.
