研究概要	結核体質:抗酸菌易感染性例の中に、抗酸菌排除に重要であるTh1経路のサイトカイン/受容体遺伝子の異常が報告されている。我々は、播種性BCG副反応を呈した10例を対象に、Th1経路に関連した分子について、IL-12受容体およびIL-18受容体の機能解析、Th1サイトカイン産生能、IFN-γ受容体1、受容体2、IL-12受容体B1、IL-12およびSTAT-1遺伝子のダイレクトシーケンスを行った。7症例については遺伝子解析を含めた各解析での異常は検出されなかったが、3症例とその父親1例においてIFN-γ受容体1遺伝子の4塩基欠損のヘテロ変異を見出した。このことからIFN-γ受容体1変異が結核体質の一つであることを示すとともに、結核体質の遺伝子診断に重要であることが示された。アレルギー体質:Th1/Th2経路に関連した因子であるIFN-γ、IFN-γ受容体1、受容体2、IFN-γのシグナル伝達に関わる関連遺伝子群およびIL-13受容体α、ヒスタミン受容体(H1、H2)、ヒスタミン分解酵素およびβ2アドレナリン受容体の各遺伝子多型についてアトピー喘息との関連性を検討した。IFN-γ受容体、IL-13受容体α、ヒスタミン受容体およびヒスタミン分解酵素の遺伝子多型にはアトピー喘息との関連は見られなかったが、IFN-γイントロン1、IRF-1およびβ2アドレナリン受容体(27Gln/Glu)の遺伝子多型についてはアトピー喘息と有意な関連を認めた。IFN-γおよびIRF-1遺伝子との関連は、アトピー喘息の発症にIFN-γおよびそのシグナル伝達が重要であり、Th1への誘導による新たな治療の可能性があることを示唆し、β2アドレナリン受容体については喘息の治療反応性を規定している可能性が示唆された。これらの遺伝子多型の解析は疾患感受性や治療反応性を予測する上で重要であると考えられた。
