動的仮想現実映像表示技術の動体迎撃放射線治療への応用研究

研究概要	(1)金マーカーの実際の座標を0.03秒毎に記録し、その動きを可視化するソフト(MathLabo使用)を開発した。マーカーの動きは(1)3次元座標表示、(2)周期解析、(3)周波数解析、(4)各X、Y、Z座標毎の時間的推移を視覚化した。これにより、20例の肺癌、10例の肝臓癌、4例の前立腺癌の体内の動きを解析し、それぞれ動体追跡同期照射により、治療精度が1-2mmまで改善することが示された。周波数解析にて、肺癌・肝臓癌の一部では呼吸性移動のみならず心拍による移動も大きな腫瘍の動きの成分であること、腫瘍の体内の位置は患者の緊張度などで変化していることが多いことが肺癌で示され、個々の患者の動きのデータに基づいた4次元治療計画を可能とした。 (2)同期照射中の線量を正確に測定し、前立腺癌5例の強度変調放射線治療(IMRT)において、同位置決め法を利用し、精度2mmでの高精度高線量治療を可能とした。 (3)昨年度開発した、「病巣座標軸に基づいた定位原体放射線治療」法を用いて治療中の病巣の回転角度を測定した。前立腺癌患者6例においても同様な計測を行い、照射中に腫瘍の縮小に伴い金マーカー間の距離が減少することが明らかにされた。 (4)腫瘍が体内で回転していた場合にも、それに対する照射角度などを補正を可能とする方法を実際の脊髄神経鞘腫の患者に用い、実空間からの情報をもとにインタラクテイブに治療装置のパラメーターを最適化することが可能となった。 (5)国際学会に(1)〜(4)が受理され、国際誌に(1)が受理され、(2)から(4)が投稿中である。
