体外肝組織再構築と組織移植による肝不全治療法の研究

研究概要

我々は、成熟ラット肝臓より肝幹細胞の一種と考えられる小型肝細胞を分離培養し、増殖と成熟肝細胞への分化を調節する方法を見いだした。小型肝細胞は増殖しコロニーを形成し、非実質細胞、特に星細胞の分泌する細胞外基質が小型肝細胞の周りに蓄積することよって成熟化し、3次元的な類肝組織を形成する。立体構造構築には、基底膜の形成が必要であり、小型肝細胞の成熟化は、非実質細胞などが分泌している増殖因子によるというよりも分泌された細胞外基質による誘導と考えられた。このことは、非実質細胞の増殖が抑制されると小型肝細胞の増殖が促進されるという結果とも一致する。分化誘導を受けた小型肝細胞は、培養後、数週間経過しているにも関わらず、成熟肝細胞が豊富に発現している転写因子のHNF4,HNF6,C/EBPalphaなどを強く発現するようになり、それら転写因子によって発現調節されているアルブミン、トランスフェリンばかりではなく、尿素合成に必要なCarbamoyl-phosphate synthetase Iやアンモニア代謝に必要なGlutamine synthetaseが発現するようになり、薬物代謝のCytochrome P450の誘導がかかるようになる。この結果は、成熟化した小型肝細胞が生体内の肝細胞と同等な機能を発揮できるということである。小型肝細胞はコロニーの状態での長期間の凍結保存(液体窒素を使うことなく-80°Cで良い)が可能で、最長90週凍結保存した小型肝細胞を増殖させることに成功した。平均60%の生着率で生着した小型肝細胞は増殖する。2週間でコロニーの面積にして約7倍に、アルブミンの産生は約5倍になる。凍結保存した小型肝細胞も成熟化を誘導することが可能であった。
ヒト肝細胞を分離し、小型肝細胞の増殖を認め、一部にラットで見られるような立体構造をとる可能性を認めた。また、ヒト肝細胞をコラーゲンからなるscaffoldの中で培養すると胆管上皮の形成が誘導され、その下で成熟肝細胞が3次元的構造を形成しつつ長期間生存した。scaffoldの中では、胆管や毛細血管の形成が認められた。
また、ラット骨髄から間葉系細胞を分離培養し、初代培養肝細胞と共培養すると小型肝細胞の増殖が促進され、成熟肝細胞の分化能の維持に良いことが分かった。
