小児短腸症候群における小腸の適応能力の解明と小腸移植の適応基準に関する研究

研究概要	これまで新生仔ブタ短腸症候群モデルを作成し、短腸症候群(SBS)に伴う肝機能障害の発生機序について肝血流、その他の側面より検討を加えてきた。実験モデルとして、7生日の新生仔ブタを、SBS群(80%小腸切除群)、機能的SBS群(バイパス手術)、Sham群(開腹腸吻合のみ)、Control群(週齢を同じくし、無処置)にわけ、術前、術後7・14日目に、血液生化学検査・門脈血流・肝組織血流を測定した。また同時に小腸および肝生検を施行し、組織像の変化を検討した。現在までにSBS群8頭、機能的SBS群3頭、Sham群2頭のデータが得られている。 1)血液生化学ビリルビン、トランスアミナーゼに群間に差をみとめなかったが、総胆汁酸はSBS群、機能的SES群で術後14日目に上昇傾向を示した。 2)肝組織血流・門脈血流肝組織血流は各群ともに手術前・後で変化が見られなかったが、門脈血流はSBS群で腸切直後から急激に減少し、その後も機能的SBS群、Sham群に比べ低値で推移した。 3)組織学的変化SBS群では8例中6例に肝障害を示す所見を得た。術後7日目は炎症細胞浸潤、術後14日目には肝細胞の脂肪変性、肝小葉の破壊、微小胆汁塞栓および活性型伊東細胞の増加がみられた。今後は、さらに長期生存例を増やし、肝組織および門脈血流と肝機能障害の発生機序について検討を加える予定である。
