研究概要

難病の克服の多くは適当な疾患モデル動物の開発によって成し遂げられてきた。現在わが国だけでも20万人以上の市民が慢性腎不全のため腎透析治療をうけていて、新たに透析治療に入る患者数も毎年3万人近くになっている。腎透析治療は慢性腎不全の内科的療法としては極めて優れた対症療法ではあるが、根治療法ではなく、大元の腎疾患を治癒させることはできない。腎透析療法は患者の増加とも相俟って莫大な医療費の増加につながりわが国が誇る国民皆保険制度をすら揺るがす程となっている。
われわれは国立感染研で発見されたネフローゼ自然発症マウスに着目し、難病のひとつである慢性腎不全の根治治療への道を探ることとした。
まず疾患モデル動物系統維持に努め、現在2つの研究機関で系統維持を行っている。微生物学的に品質の高い疾患モデル系統を維持できたことから、海外2研究機関、国内8研究機関に系統を分与することができた。続いてモデル動物の遺伝様式を明らかにし、責任遺伝子を解析したところ本疾患は常染色体性劣性遺伝していることが明かとなっていたが、その主責任遺伝子は第15番染色体上にmapされ、さらに詳細な遺伝子部位が明らかにされた。すでにマウスでは全遺伝子のシーケンスが公表されたことから、本モデルの主遺伝子の本体もまもなく解明される見通しである。さらに病態解析が進み慢性腎不全の本態は尿細管間質の繊維化であることが明かとなり、これはヒトにおける慢性腎不全病態解明の最近の成果とまったく矛盾しないことが判明した。すなわち本モデルがヒト疾患モデルとして極めて好都合であることを示すことができた。これをもとに尿細管間質繊維化の予防および解消が治療戦略上有用である可能性が高まり、繊維化に関与するサイトカインの解析を進めた。この経過からサイトカインの遺伝子治療が慢性腎不全には適していることが示唆され、現在、遺伝子治療の研究が本モデルを用いて行われている。
