研究概要

本研究成果の概要は下記の4項目としてまとめられる。
エキノコックス症(多包虫症、単包虫症)は平成11年4月1日から施行されている感染症予防新法によって届け出が義務づけられた第4類感染症に含まれている。この法律の施行下で本州から2症例の多包虫症が届けられているが、第1症例は肝蛭症(秋田)第2症例は単包虫症であり、国内で流行が問題になっている多包虫症ではなかった。このような誤診を防ぐ意味からも旭川医大の研究は緊急を要するテーマである。血清学的に鑑別が必要な寄生虫疾患には、多包虫症、単包虫症、有鉤嚢虫症とがあるが、これら3種類の寄生虫疾患についての血清学的鑑別診断法は旭川医大ですでに確立され、国際的にも最も信頼性が高い検査法であると評価されている。本研究では特に多包虫症についての国際標準診断法、早期診断法、治癒判定法の実用化を目的とする研究を推進している。1.Em18抗原を用いる血清診断、血清疫学への展開:中国、四川省、青海省、甘粛省を中心に、Em18-immunoblot陽性者は多包虫症であり、鑑別確定診断法として重要であると評価された。2.早期診断にEm18immunoblotが利用可能かどうかについては、これまでの国内での検査において陰性であった多包虫症例も検出できていることから、早期診断にも利用可能であると予測している。3.イムノブロット用抗原として粗Em18抗原、精製Em18抗原の2種類を用いている。さらに、特異抗体を用いたAffinity精製Em18抗原を用いている。特にAffinity精製Em18抗原を用いている場合に、感度がこれまでよりもかなり高くなることが判明している。4.治癒判定法としては精製Em18抗原、遺伝子組換え技法によって作製しているprototype recEm18(rEMAG5)を用い、ほぼ類似の経過観察により予後判定が可能であるという成績が得られている。
