研究概要	本研究における主要研究テーマは、現在国内で北海道の風土病として、第2の肝臓癌と呼ばれるエキノコックス症に関する信頼性の高い免疫診断法の開発と実用化への展開である。旭川医大で報告してきたエキノコックス症(多包虫症)血清診断抗原Em18について、これを特異抗体を用いるアフィニティー精製法と遺伝子工学の2通りの方法により、格段に信頼性が高い抗原作製と検査の簡便化に成功している。本年度の研究成果の第1は遺伝子組換えEm18(RecEm18)の作製である(Sako et al. in preparation)。このRecEm18を用い、(1)北海道内の関連病院を中心とするエキノコックス症血清学的再検査(基本的には行政主導で行われたエキノコックス症血清検査成績と旭川医大での検査結果についての比較)、(2)旭川医大で経過観察中のエキノコックス症症例、(3)フランスとの共同研究としてのブラインドテストによるEm18抗原の診断抗原としての信頼性検討、(4)中国におけるエキノコックス症疫学調査成績についての血清学的検討を試みてきている。 RecEm18作製法とその診断における評価については現在、論文が完成し、投稿準備中である(Sako et al. Journal of Clinical Microbiology submitted)。 (1)については最近相談を受けた30症例中、5例が現行の他の検査機関による誤診検査であると結論された。この成績を含めEm18を用いる血清診断法についての総説を発表した(Ito et al. 2001 ; Ito et al. 2002a. Acta Tropica ; Ito et al. 2002b. in Cestode Zoonoses(IOS Press)、その他和文論文多数)。 (2)については現在、臨床講座で論文作成中で、(3)については論文が完成している(Ito et al. Emerging Infectious Diseases submitted)。(4)については中国四川省における多包虫症、単包虫症、両症が重複流行地における地域住民の検査を現在、試みている段階である。これらの研究実績に基づき、エキノコックス症対策指針総説を外国の研究者達と幾つかまとめている段階である。来年度は、これまでに作製、精製に成功しているRecEm18を用いるキット化へと研究が展開するものと予測している。
