作業関連疾病予防を目的としたHLAタイピングによる有害物質感受性の判定

研究概要

作業関連物質への曝露によって、種々の疾病が起こることは周知の如くである。珪酸や珪酸塩化合物による健康障害もすでにギリシャ・ローマ時代から良く知られているところである。私共は、珪肺症患者の血清を調べたところ、その70%以上に自己抗体が検出された。新しく確認された自己抗体には、topoisomerase I,β-tubulin, Fas, caspase-8,caspase-9等に対する抗体や、ANCA, pemphigus抗体等も含まれることが判明した。職業的に原因物質への曝露が避けられないものであるならば、これらの曝露によって自己抗体の産生や自己免疫疾患に至る人と、特に変化を示さない人との、遺伝的素因を比較して、種々のリスクファクターを見出すことにより、予防医学的にリスクのある現場への配位を原因物質と素因との関係から工夫することにより、職業に由来する疾患を可能な限り避けることが出来るのではないかと考えて、本研究を行って来た。この素因の一つとして注目したのがHLAタイピングである。HLA class IIの型は、インシュリン依存性糖尿病をはじめ種々の自己免疫疾患に関連していること、絶対的に優性又は劣性のHLA型が知られていないことから、HLA型タイピングが社会的不利益を伴う可能性が今のところ少ないことが理由として挙げられる。抗-topoisomerase I自己抗体陽性者を例に挙げると、日本人ではHLA DQB1に^※0402など^※04を持つ人が多く、白人では異なったalleleと相関すること、これらの共通点を調べるとDQB1ドメインの14番目のアミノ酸がメチオニン、30番目がタイロシン、(57番目がアスパラギン酸)、77番目がスレオニンであることが判明した。一方、この群では比較的重要性を持たない57番目のアスパラギン酸の変異が抗一casapase-8自己抗体では強い関連性を示し、DQB1*0302が陽性者に多くみられ、インシュリン依存性糖尿病と類似していた。以上の如く、新しく検出された自己抗体各々についても詳細な解析を行って来た。更にデータを加えてこれらをリスト化することにより、就業時の適性判断と作業関連疾患予防に役立ちうるものと考える。
