HLA-DPB1分子結合モチーフからみた視神経脊髄型多発性硬化症特異抗原の同定

研究概要	私どもは、アジア人種に高率に認められる視神経脊髄型多発性硬化症(OS-MS)の疾患感受性遺伝子がHLA-DPB1^0501遺伝子であること、また、主要なミエリン抗原(MBP, PLP, MOG)に反応性を示すT細胞株を樹立した場合、OS-MSと通常型MSとでは、エピトープの拡大する傾向が抗原蛋白により異なることを報告してきました。近年、MSの主要な標的であるオリゴデンドロサイトの起源は単一でなく、視神経・脊髄と大脳ではlineageが異なる可能性が指摘されており、OS-MSの病巣の形成には、視神経・脊髄に特異的な自己抗原が存在する可能性が考えられ、以下の研究を開始した。平成12年度、ヒト脊髄cDNA発現ライブラリーを作成し、SEREX法にてOS-MS患者血清中に存在する自己抗体の認識する自己抗原のスクリーニングを開始した。OS-MS患者8名の血清(1名の血清に対しおよそ5×10^5〜1×10^6個のファージのスクリーニング)より11の陽性クローンが検出され、塩基配列を決定した結果、5つの候補自己抗原が同定された(Rabaptin-5,HSP105,NSD1,EST(KIAA0610,KIAA1640))。平成13年度、候補自己抗原の内、Rabaptin-5,HSP105について、リコンビナント蛋白を作成し、ELISA法を用いてMS患者、健常対照にて抗体価を測定したところ、抗Rabaptin-5-IgG抗体は、MS群で有意な陽性率の上昇認められなかったが、抗HSP105α-IgG抗体は、MS群において血清ならびに髄液において有意な上昇が認められた。しかしながら、OS-MSと通常型MSとの間においては有意な差は認められなかった。HSP105に対する自己反応性T細胞の検討では、HLA-DPB1^0501を有するMS群において反応性の亢進が示唆された。以上のことより、SEREX法にて単離しえたHSP105が、MSの新規自己抗原であることが示された。今後更に、NSD1、EST(KIAA0610,KIAA1640)についてもMS患者における抗原性を検討していく予定である。
