腫瘍特異的代謝亢進に基づく癌の内部照射治療を可能にするヨウ素標識薬剤の開発

研究概要

本研究では、腫瘍特異的代謝亢進に基づき腫瘍組織に集積・滞留する新規ヨウ素標識薬剤の開発を目的とする。特定の腫瘍細胞系における滞留性の向上を目的とした本研究成果は、癌の内部照射治療薬の薬剤設計に新たな視点を与え得る。
悪性黒色腫は極めて予後不良の疾患であり、その特異的診断法及び治療法の開発が望まれている。黒色腫細胞ではメラニン形成が異常に亢進しており、フェノール環を有する含硫アミン誘導体はメラニン生合成に必須のチロシナーゼとの基質親和性が高いことから、チロシナーゼ基質類似体を利用したヨウ素標識人工アミノ酸(I-L-PC)を合成した。その結果、B16黒色腫担癌マウス体内分布において、黒色腫組織への高い集積及び滞留性が確認された。また、チロシナーゼ阻害剤による集積低下が顕著であることから、I-L-PCのチロシナーゼとの相互作用が示唆された。
さらに、メラニン生合成経路への親和性の向上を期待して、新たに含硫アミン誘導体(4-PCA)を考案した。4-PCAは合成により得た。その放射性ヨウ素標識体(I-PCA)は、高い標識率及び放射化学的純度で無担体標識体として得られた。B16黒色腫細胞を移植したマウス体内分布では、I-PCAはいずれの正常組織への集積も低く、血中からの消失は非常に早く、早期に体外排泄された。一方、黒色腫組織への集積は軟部組織と比較して高く、滞留性も認められた。その結果、画像化に必要な対周辺組織比は既に臨床使用されている腫瘍診断薬を上回っていた。
一方、腫瘍診断薬を含む放射性医薬品の中には、血漿蛋白への結合により組織移行性が妨げられているものがある。そこで、蛋白結合部位において結合置換効果を示す安全性の高い薬物を併用し、血中遊離濃度を増大させることにより、組織への移行性を高め、更に体外への排泄を促進させることを試みた。その結果、蛋白結合性の高いヨウ素標識薬剤I-IMPに関して、血漿蛋白上の特異的結合部位を同定すると共に当該部位で競合阻害効果を示す安全性の高い置換薬を併用し、組織移行性の決定因子である血中遊離濃度を顕著に増加させ得ることを確認した。さらに、同様の置換薬を投与したサルにおいて、I-IMPの脳集積量が無負荷時の1.4倍に増加するなど、医薬品の標的組織への集積性を向上させ、同時に体外排泄を促進させ得ることを実証した。
