研究概要

単球・マクロファージが産生するIL-12はINF-γの産生を促進することにより、Th2タイプの反応(IL-5により誘導される好酸球性炎症など)を抑制する。逆にIL-10は単球、マクロファージによるIL-12産生を抑制し、Th2システムの優位を誘導する。わが国を含め先進国において、アトピー性疾患が増加しているのは、予防接種や抗生物質の開発によりTh1システム優位を誘導するウィルス感染や細菌感染が減少しているせいかもしれない。実際、そのような報告もある。アトピー性疾患の病態の研究においてヒト(human : h)IL-10とDNA配列で84%の相同性の見られるEBVのBCRF1遺伝子産物の存在が発表され、viral IL-10(vIL-10)と名付けら、EBVはアトピー性疾患の発症において何らかの重要な役割を果たしている可能性がある。本研究において我々は、気管支喘息、アトピー性皮膚炎患者、無症候者、正常者の血液標本総計226例を採取し、総IgEレベル、ヤケヒョウヒダニ、コナキョウヒダニ、日本杉、卵白、牛乳、大豆、イヌ、ネコ等に対するIgE抗体価、総白血球数、好酸球パーセンテージなどアトピー性疾患のマーカーを測定し、同時にEB viral capsid antigen(VCA)に対するIgG, IgM抗体価およびEBV nuclear antigen(EBNA)に対する抗体価を検査してEBウィルス感染とアトピー性疾患発症の関連を検討した。その結果、幼小児期(0-10歳)における抗EBV抗体価の上昇がアトピー性疾患発症と密接に関連することが判明した(P<0.01)。これらの所見はアトピー性疾患の発症に遺伝的要因より環境的要因、特にEBV感染が重要であろうという我々の主張を裏付けるものである。
