若年発症肺癌(30歳未満)における分子生物学的特性と遺伝子異常の検討

研究概要

前年度に引き続き当教室で肺癌登録912例の背景(家族歴における癌患者の有無,重複癌の有無,喫煙歴,職業歴)を調査が終了し,第二外科の症例を追加検討中である。30歳未満の対象は男性4例,女性3例の計7例であり,組織型は腺癌3例,小細胞癌2例,扁平上皮癌1例,臨床病期はIIIA期1例,IIIB期2例,IV期4例で(対象を40歳未満とすると男性9例,女性8例)であった.集積した症例を中心に随時HE染色・各種免疫組織染色(CAM5.2:Cytokeratin8/18,involucrin, surfactant protein A, CEA, synaptophisin, neurofilament, CD45, CD56, CD34, P53)を実施し,現在解析中である。
24歳の肺小細胞癌患者から樹立された細胞株SBC-3,20歳の肺扁平上皮癌愚者から樹立された細胞株EBC-2を利用して,細胞株自体の遺伝子異常と患者germlineの異常を(EBC-2のみ)検討中である.SBC-3のp58 gene statusはexon 4 motility shift by SSCP without mutation (heterozygous), exon 5 motility shift by SSCP with mutation (heterozygous) codon175 CGG to CAC (Arg to His), p16で検討したexon1A,1B,2A,2B,2C,3に異常は認めなかったが,pRBの過剰発現が認められた.他の遺伝子異常についても検討中である.Fow Cytometryの検討ではSBC-3,EBC-2ともにCD34を発現していた.また,EBC-2には電子顕微鏡で扁平上皮癌に典型的なdesmosomeとtonofilamentを認め,薬剤耐性遺伝子MRP1,3,4,5,LRPの発現は認められたが,MRP2,P-glycoproteinの発現は認められなかった(Lung Cancer,35(3):305-314,2002).EBC-2のsublineであるEBC-2/Rはcsiplatinに対して耐性を示すが,EGFRのtyrosine kinase inhibitorに対して感受性を示し,tyrosine kinaseのリン酸化には変化がなくPI3K-AktとのMEK-MAPK signals関与が推察され,現在解析中である。
