肝類洞側および胆管側への肝細胞内脂質移動における指向性制御機構とその臨床的意義

研究概要

肝内・肝外胆汁うつ滞モデルにおいて、肝類洞側膜および肝毛細胆管膜の脂質二重層の脂質組成および胆汁脂質組成、膜脂質輸送担体の間に以下の興味深い関係を見出した。
1.肝細胞膜脂質二重層の脂質配列と胆汁脂質には相補性があり、肝細胞機能恒常性の維持機構・組織保護機構としての位置付けが想定される。特に、スフィンゴ・リン脂質は膜構築の強度や機能性の維持に寄与していると考えられる。
2.肝細胞膜脂質分子種の変化は膜流動性に関与しており、構成リン脂質の疎水性強度が上昇すると膜構築は安定化するが、流動性は低下する。逆に、親水性リン脂質分子種が増加すると膜流動性は高まるが、膜構築は脆弱化する。
3.内因性胆汁うっ滞では膜局在ABC transporterの発現は低下し、膜流動性も低下して胆汁排泄能は速やかに低下する。一方、局所的外因性胆汁うっ滞では健側肝の代償機構が成立すること、全肝における外因性胆汁うっ滞では胆管側と類洞側の輸送担体の間に相補性が成立することが明らかとなった。即ち、これらの代償機構が肝に潜在する機能的予備力の一端を担うことが示唆された。
