転移性消化器癌ワクチン開発の基礎的研究

研究概要

上皮癌患者末梢血より樹立した細胞傷害性T細胞(KE4-CTL)を用いて新たにクローニングしたlckは、リンパ球系細胞の増殖・機能発現に関与する一方、転移性大腸癌や肺癌の一部でも異所性に発現している事から本分子由来のペプチド抗原が転移癌と関連した癌ワクチンの候補になり得るかどうかを検射した。lckの発現は正常細胞・組織ではType IIプロモーターが関与しているのに対して、癌細胞では主にType Iが関与していることがRT-PCRの結果で確認され、タンパク質レベルでは両者共同じ易動度のタンパク質としてウエスタンブロットで検出された。タンパク質の発現は正常細胞でも認められたが、KE4-CTLは認識しないことが示唆された。しかし、CTLが正常細胞は認識せず癌細胞を選択的に傷害する機序に関しては現在のところ明らかではない。HLA-A24結合モチーフを有する合成ペプチドを、C1R/A*2402細胞株と反応させた後KE4-CTLが産生するIFN-γを測定し、HLA-A24拘束性に認識するペプチド抗原を3種類(Lck208-218,Lck486-494およびLck488-497)同定した。次に決定したペプチドで患者末梢血細胞を刺激し、KE4細胞株に特異的なCTLの誘導を試みたところ、転移癌患者ではペプチド刺激していない群でも前駆細胞が認められ、刺激することにより増強されることが示唆されたが、非転移癌患者および健常人では認められなかった。以上の実験成績から、本ペプチドが、特に転移癌の癌ワクチン候補となりうることが示唆された。現在共同研究者の伊東らが、Lckを含む我々の研究室で同定した複数のペプチドのなかから患者末梢血中の前駆細胞をあらかじめ検査してからワクチンを投与するという、50例以上の高度進行癌を標的としたペプチドワクチンの臨床試験を行っており、Lckペプチドは80%以上に投与されている。また、投与部位における発赤以外の有害事象は現在までのところ認められていない。
