QT延長症候群内Molecular Medicine-遺伝子診断を基盤とした治療を求めて

研究概要	我々は、これまでに比較的若年者に見られる心臓突然死の原因として近年注目されている家族性及び2次性QT延長症候群(以下LQTS)の患者を対象として、心筋イオン・チャネルにおける遺伝子変異の検索し、患者の臨床像と、同定された変異が招来するところのチャネル機能障害との相関関係について検討してきた。これまでに100以上の症例について、発端者とその家族に対して遺伝子検索を進めた結果、9家系で8つのKCNQ1遺伝子の変異を、6家系で6つのHERG遺伝子の変異を、2家系で2つのSCN5A遺伝子の変異を同定した。特に、カリウムチャネルをコードするKCNQ1とHERGの遺伝子変異についての検討において、チャネル蛋白における部位からみたとき、家族性LQTSで同定された変異は、主に機能的により重要な細胞膜貫通領域とイオンの通過する孔領域に存在する傾向が認められたのに対して、2次性LQTSで同定された変異は、細胞内リンカーやC末端領域に存在していた。それらの遺伝子変異について、イオンチャネル異常を機能解析したところ、前者の変異は、それらの導入によってチャネル機能が廃絶してしまうのに対して、後者の変異は、主に軽度の機能異常を引き起こすものであった。これらの結果は、患者の臨床的重症度に概ね対応しており、遺伝子検索の臨床的意義が示唆された。
