Tilt Training:神経調節性失神の新しい治療法の開発と病態機序解明

研究概要	背景:Head-up tilt試験で誘発される神経調節性失神の治療法は薬物治療とorthostatic self-trainingが有効である。目的:1)神経調節性失神に対する薬物治療とorthostatic self-training治療の有効性を比較検討し、また2)orthostatic self-training治療の有効性の機序と3)長期成績について検討すること。対象・方法:Head-up tilt試験で神経調節性失神と診断された患者45名。1)薬物治療(交感神経ベータ遮断薬とDisopyramide)を19名に、orthostatic self-training治療を24名に施行し、HUTでその有効性を比較検討した。2)orthostatic self-training施行前後でHUT時の心電図より心拍変動解析を行った。3)orthostatic self-training治療(一日一回、30分間)のみで失神発作の再発につき長期経過観察を行った。結果:1)薬物治療のみのHUTによる有効性は50%以下であったのに対し、orthostatic self-trainingの有効性は90%以上で有意に高かった。2)心拍変動解析の結果、orthostatic training後の立位早期においてL/H比(交感神経機能)亢進の有意な低下を認めた。HF比(副交感神経)はトレーニング前後で変化を認めなかった。3)平均9.5ヶ月の長期観察においてorthostatic training治療群では失神発作の再発は認めなかった。総括:orthostatic self-training治療は薬物治療に比し遥かに有効な治療法であり、長期観察においてもその有効性が確認された。本治療法は立位早期の交感神経機能の過剰な亢進を抑制することがその機序として考えられた。
