ドーパミン及びセロトニン受容体遺伝子型の薬力学的指標への応用性に関する研究

研究概要

ドーパミン受容体遺伝子多型であるTaqI A遺伝子多型および-141 Del/Ins遺伝子多型について、以下の関連を検討し、成果を報告した。
1)TaqI A遺伝子多型と抗精神病薬投与中に起こる重篤な副作用である神経遮断薬悪性症候群との関連を検討した。神経遮断薬悪性症候群の既往を有す患者群では、対象患者群と比較して、ドーパミン受容体密度や機能の低下と関連するA1遺伝子保有率が有意に高かった。本所見より、神経遮断薬悪性症候群の発症機序に遺伝的に規定されたドーパミン神経系の脆弱性が関与し、TaqI A遺伝子多型の同定が神経遮断薬悪性症候群の予測因子となる可能性が示唆された。
2)TaqI A遺伝子多型と抗精神病薬治療中のプロラクチン反応との関連を検討した。ドーパミン受容体密度や機能の低下と関連するA1遺伝子を有す女性では他の群と比較して有意にプロラクチン反応が高く、A1遺伝子保有女性は抗精神病薬投与中に起こる高プロラクチン血症に関連した副作用のリスクが高い可能性が示唆された。
3)-141 Del/Ins遺伝子多型と抗精神病薬の治療効果との関連を検討した。ドーパミン受容体密度の低下と関連するDel遺伝子非保有者では、分裂病の周辺症状である不安-仰うつ症状の改善率が有意に高かったが、全体の治療反応性はDel遺伝子の保有者群-非保有者群との間に有意差はなかった。本結果は、-141 Del/Ins遺伝子多型が抗精神病薬の抗不安作用や抗うつ効果に影響を及ぼすことを示唆する。
以上より、TaqI A遺伝子多型は抗精神病薬治療中の一部の副作用発現に関与し、-141 Del/Ins遺伝子多型は抗精神病薬の治療効果を部分的に修飾する可能性が示唆された。
