非アルツハイマー型変性痴呆の病態機序に関する研究-細胞骨格蛋白異常の観点から-

研究概要	「非アルツハイマー型変性痴呆の病態機序に関する研究-細胞骨格異常の観点から」のうち、本年度はレビー小体型痴呆(DLB)の細胞病理学的研究と第17番染色体に連鎖する前頭側頭型痴呆(FTDP-17)の細胞病理学的・分子遺伝学的研究を行った。まず、DLB脳に特徴的な神経細胞内封入体であるレビー小体の変性機序について、レビー小体の線維構造物の主な構成成分であるα-シヌクレインとグリア関連抗原および炎症・免疫関連抗原を用いた免疫組織化学と免疫電顕により検討した^<1)2)>。その結果、レビー小体含有神経細胞の変性に伴い補体蛋白やクロモグラニンAなどのミクログリア活性化因子が陽性となり、その後活性化ミクログリアが変性したレビー小体含有神経細胞に集族し、炎症性サイトカインの分泌を伴って神経細胞死に導くことが示唆された。さらに、細胞外に出たレビー小体はα-シヌクレインを失い、アストログリアの修飾を受けて長期に渡り細胞外に止まることが示唆された。また、FTDP-17は最近注目されているタウ遺伝子異常を伴う家族性前頭側頭型痴呆であり、我が国でも数家系が報告され、多彩な臨床病理学的所見を示すが、今回新しいタウ遺伝子変異をもつFTDP-17の剖検例を報告し、神経細胞と比較してグリア細胞により広範な燐酸化タウの蓄積がみられ、とくに大脳皮質のアストログリアでは従来報告されている異常線維構造物を欠き、増生したグリア細線維に燐酸化タウの蓄積をみたのが特徴的であった^<3)>。
