アデノ随伴ウィルスベクターを用いた血友病Aに対する遺伝子治療の基礎的検討

研究概要	ヒト第VIII因子を搭載するベクター構築に先立ち、骨格筋に対するAAVベクター投与法の基礎実験として、至適プロモーター及び血清型の検討を行った。肝臓に対してはCMV、CAG、PGK、EF-1α等のプロモーターを用い、マウス由来のエリスロポエチン(Epo)cDNAを搭載したベクターを構築・作製した。以上のAAVベクターを経門脈的に投与し、遺伝子発現効率を血清エリスロポエチン濃度及び血液学的指標を用いて系時的に比較した。その結果、CAG、CMV、PGK、EF-1αの順に強い発現が得られた。至適と考えられるCAGプロモーターでマウスEpoを発現するユニットを用いてAAVの1型から5型までの血清型の違いによる遺伝子発現を比較検討した。その結果、5型を用いた場合に最も高い発現が見られた。骨格筋に対しては実績のあるCMVプロモーターを使用し骨格筋内への注入による血清型の比較を行ったところ、1型が最も高く、5型がこれに次ぎ、それ以外が2型とほぼ同程度とする結果が得られた。以上の結果は長期にわたる観察によっても同じ傾向を示した。以上の結果をふまえてヒト凝固第VIII因子遺伝子を搭載したAAVベクターの構築を行った。第VIII因子遺伝子を重鎖と軽鎖を二つのベクターに分けて標的細胞に共感染させることで発現を目指している。肝臓に対してはCAGプロモーターと5型キャプシドを用いて、また、骨格筋に対してはCMVプロモーターと1型キャプシドを用いて各々作製した。このストラテジーの有用性はin vitroの実験系で確認が出来たため、in vivoで検討を行うべくベクターの大量作製を行った。凝固第VIII因子ノックアウトマウスも既に入手できており、現在注入に向けた準備を行っている。今後は注入と共に経過観察、治療効果の確認を行い、有効であれば更に大型の動物における治療実験を指向する予定である。
