小腸移植における成長因子の腸適応促進効果に関する研究

研究概要	小腸移植は機能的又は物理的に小腸の機能不全があり、経腸栄養はもちろん経静脈栄養によっても生命維持が不可能な症例に対し適応となっている。現在移植手技と免疫抑制剤の進歩によって、移植後の経過は改善してきたが、満足できる状況とは言い難い。これまでに我々はラット小腸移植後にEpidermal growth factor(EGF)を用いて検討を行ってきた。これまでのラット同系2期小腸移植では陰窩長のみにEGF投与群が有意な差を認め、異系1期小腸移植ではEGF投与群は絨毛高にのみ有意な差を認めたが、今回のラット異系小腸移植群を2期モデルで作成するとEGF投与群は絨毛高及び陰窩長でも有意に非投与群に対し増加していた。また、ナトリウム-グルコース共輸送体(SGLT1)やジペプチド共輸送体(PepT1)のタンパクレベルの誘導や二糖類分解酵素活性はEGF投与群が非投与群に対して有意な差を認めた。SGLT1の誘導がEGF群は増加していることに関連し、糖吸収効果も細胞レベルにおいては増加していた。これまでの糖吸収に対する検討では、移植腸管の灌流試験を行ってきたが、灌流試験では精度に問題があると判断した。今回は移植腸管の粘膜細胞のみを遊離し、3-O-メチルグルコースにRIを付加し、細胞培養中に添付することにより細胞内へのグルコースの取り込みを検討した。これによりEGF投与群は非投与群に対し有意に糖の取り込みが増加していることが確認された。また、1期手術と2期手術ではモデルに対する侵襲が異なることを報告しているが、絨毛高と陰窩長のEGFによる増加はこの傍証になると考える。今後は他の成長因子として期待されているホルモンなどを検討し、より臨床に還元していきたいと考えている。
