膵癌肝転移に対するp53アデノウイルス・ベクターを用いた遺伝子治療に関する研究

研究概要

膵癌はたとえ切除可能であったとしても、局所再発ないしは肝転移ないしは肝転移による再発のためその手術予後は極めて不良である。局所再発に対しては拡大切除や術中放射線照射などによりある程度の制御効果が得られているものの、肝転移に対しては化学療法の有効性も乏しく、遺伝子治療をはじめとする新たな治療法が期待される。
本研究の目的は、膵癌の肝転移に対するp53 adenovirus vector経門脈的投与の有効性を評価することである。マウスを用いた動物実験の結果、p53遺伝子の変異を有する腫瘍の肝転移においては、正常型p53 adenovirus vectorの門脈内投与の抗腫瘍効果が確認された。また、マウスにおいては正常型p53 adenovirus vectorの複数回投与も可能であり、複数回投与による抗腫瘍効果の増強も認められた。一方、p53遺伝子の変異を有しない腫瘍の肝転移においては、正常型p53 adenovirus vectorの門脈内投与の抗腫瘍効果は確認できなかった。尚、正常型p53 adenovirus vectorの門脈内投与による明らかな副作用は認められなかった。
さらに、肝転移に対する癌遺伝子治療の臨床応用への可能性を広げるため、上記モデルを用いて、IL-12を遺伝子導入した線維芽細胞を門脈内投与する治療法の抗腫瘍効果ならびに抗腫瘍免疫の誘導に関しても明らかにした。今後、肝転移に対する癌遺伝子が臨床応用され、癌患者の予後ならびにQuality of Lifeの改善されることが期待される。
