研究概要

Peutz-Jeghers症候群(PJS)9家系10人の患者から得られた27個の過誤腫性ポリープについて、原因遺伝子であるLKB1遺伝子の生殖細胞変異を解析するとともに、LKB1遺伝子体細胞変異およびその他の癌抑制遺伝子や癌遺伝子(APC,p53,β-catenin,K-ras)の体細胞変異を解析した。LKB1遺伝子の生殖細胞変異がPJS6家系で検出された。LKB1遺伝子の体細胞変異が5個のポリープで検出され、19pLKB1 locusのLOHが他の14個ポリープで検出された。大腸と胃のポリープ内に存在する腺腫部分にβ-catenin mutationと19pLOHが検出された。また、胃ポリープの癌部分ではLKB1 locusの19pLOHとβ-catenin mutationと共にp53 mutationとLOHが検出された。K-ras mutationは数個のポリープで検出されたが、APC mutationと5qLOHは検出されなかった。PJS患者の消化管過誤腫性ポリープが生殖細胞変異プラス体細胞変異あるいはLKB1 locusのLOHによって発生し、β-cateninとp53遺伝子のmutationが加わり癌に進展することが示唆された。
